If you are not a Metropolitan Hospital Affiliated Physician, you will need to obtain privileges through their Medical Board. Our Physician Coordinators can help you with this process. The Metropolitan Medical Board will require the following documents for your application:
- Current Curriculum Vitae (CV)
- Copies of your:
- DEA Certificate
- NYS Medical License (including signed NYS License Registration Certificate)
- Board Certificate or Evidence of current board status or letter of admissibility
- All Professional Training Certificates (i.e. Medical School Diploma, Residency Certificates, Fellowships, ECFMG Certificates, etc.)
- Life support certification (The application will say BLCS; however BLCS will not be required)
- Infection Control training Certificate from NYSDOH approved training program
- Evidence of current professional liability insurance coverage, if applicable (copy of face sheet)
- Proof of Identity

Please indicate if you speak Spanish fluently and if you are familiar with the Metropolitan Hospital Computer System (QuadraMed).
If you would like to volunteer with the Clinic Operations/Administration, you can do so by contacting any of the Executive Board Members.
